Deceased Documents

1. Three (3) certified copied of National ID card.
2. Three (3) certified copies of death certificate
3. Current payslip
4. Doctors’ death confirmation letter (BMD1)
5. Body confirmation letter from mortuary

Beneficiary Documents

1. One (1) certified copy of national ID card
2. Bank statement

NB: All Document must be submitted to the office within 6 months of the deceased date.
